Once your appointment is scheduled, you will receive a packet of paperwork in the mail. Please complete the enclosed forms and bring them with you to your appointment.

 

For your convenience, you may print out the forms from our online "common forms" section, fill them out and bring with you to your appointment.

 

Office Appointment Policy

Our office appointments include patients scheduled to see surgeons and/or Nurse Practitioners in consultation for surgery, as follow-up to surgery and/or for diagnostic studies in our Vascular Laboratory. Because of our various services, appointment lengths may vary. Patients are not necessarily seen in order of arrival.

 

We apologize if you have to wait to be seen. However, cardiac, vascular and thoracic surgical care can be urgent in nature, and it is our practice to treat patients according to priority.
